This document is required only for Medical Assistance (Medicaid) clients and PMAP (Pre-paid Medical Assistance Program) ADULT clients. Please print this with your intake paperwork and bring into your first session. 

This form is required only for all Optum Insurance clients (Medica, United Behavioral Health).
